The Trump administration just proposed cutting off all Medicare and Medicaid payments to any hospital that provides gender-affirming care to minors. NPR got the draft. If it goes through, hospitals treating trans kids lose access to every federal dollar they depend on – for everything from cancer treatment to emergency care. One trans patient, and the entire hospital is blacklisted.

It hits hardest where poor families live. Medicaid and CHIP cover low-income kids. So when a trans teen on Medicaid needs care, the hospital faces a choice: treat the patient and lose funding for everyone, or turn the kid away.

And because no hospital can survive without federal reimbursement, most will stop offering gender-affirming care entirely, even to patients with private insurance. One regulation becomes a nationwide ban that Congress never had to vote on.

Two dozen states already ban this care. This rule would erase the protections in the rest – California, New York, Illinois – doesn’t matter. If the hospital takes federal money, the care stops.

Every major medical group – the American Academy of Pediatrics, the AMA, the Endocrine Societyv- supports gender-affirming care for youth when clinically indicated. Its evidence-based, not experimental. The data are clear: access to this care lowers depression and suicide risk. Denying it does the opposite.

The rule is still in draft form. There will be public comment and lawsuits. But the strategy is obvious: use the budget to achieve what legislation can’t. Make the care impossible to access, then pretend it was never banned.

The cruelty isn’t incidental. Its the whole design.
